PDF order form (blank - to handwrite and fax to 801-569-8225) PDF order form (fill-in form on your PC)   Instructions:: Either order form will load into a new window. Please allow a few moments for it to load, especially the fill-in form (which requires the free Adobe Acrobat reader which is probably already installed on your computer). The blank form is intended to simply be printed, handwritten and faxed. If you are using the fill-in form, press TAB to start (or use your mouse), enter your phone number and proceed to enter TAB or use your mouse to navigate and enter your address and order information. Enter all desired quantities. If you are a Utah resident and/or are subject to Utah sales tax, click or mark the Taxable check box. Assuming that you are using the Adobe reader (may not work with some readers such as the Foxit PDF reader/plug-in), the fill-in PDF will automatically calculate once you leave a field. If a quantity is entered or changed and the total has not changed, simple click somewhere else on the form. Enter the payment details (if credit card, enter all required information). When done you can either (a) click on the Print icon and from there you can either print the order to your printer and fax to us, or if you have a fax modem, select your fax printer driver and fax without printing (our fax number is 801-569-8225) or (b) click on the Save (picture of a disk) icon and save the PDF to a folder on your local drive and then e-mail the saved PDF to prolix@prolixlubricant.com. When done, simply close the order form window.
